Chapter Connection Survey Contest
No purchase is necessary to enter or win. A purchase will not increase your chances of winning. All Federal, State, Local, and Municipal laws and regulations apply. Void where prohibited. The one-thousand-dollar prize ($1,000) will be paid to the winner’s WoodmenLife chapter.
Eligibility: The Chapter Connection Survey Contest (the “Survey”) is open to Woodmen of the World Life Insurance Society (“WoodmenLife”) members in good standing who are at least eighteen (18) years old at the time of entry. WoodmenLife Sales Representatives, Regional Sales Managers, Regional Directors, Home Office Associates, Community Outreach Advisors, and Regional Office Administrative Assistants and their immediate family (spouse, parents, siblings, and children) and household members of each such WoodmenLife associate are not eligible. This promotion is not open to New York residents.
Sponsor: The Contest is sponsored by WoodmenLife, located at 1700 Farnam St., Omaha, NE 68102.
Agreement to Official Rules: Participation in the contest constitutes entrant’s full and unconditional agreement to and acceptance of these Official Rules and the decisions of WoodmenLife, which are final and binding. Your chapter winning a prize is contingent upon being compliant with these Official Rules and fulfilling all other requirements set forth herein.
Contest Period: The Contest begins on October 2, 2023, Central Daylight Time and ends on November 3, 2023, Central Daylight Time (the “Contest Period”). Entries that are submitted before or after the Contest Period will be disqualified. Submissions will be accepted for the duration of the contest using any of the following methods: Mail-In and Online.
How to Enter: There are two (2) ways to enter. 1. Online – This method of entry will be available by visiting our website https://forms.office.com/r/mTfd3xUAWB and following the directions provided to fill out the entry information and submit. For each Survey submitted with a valid first and last name, the member receives one (1) entry into the drawing. Members can only be entered once during the Contest Period. 2. Entry by Mail – This method of entry will be available for the entire Contest Period. On a piece of paper, print and complete the Survey and mail in an envelope with proper postage affixed to: WoodmenLife Contest, WoodmenLife PO Box 35903, Cleveland, OH 44135. All mail-in entries must be postmarked by November 3, 2023, and received by November 10, 2023. Limit one (1) mail-in entry per member for the duration of the Contest Period. Entries received from any person, email address or household in excess of the stated limitation will be void. All entries become the property of WoodmenLife and will not be acknowledged or returned.
Valid Entry: A Survey must be completed in good order. The member must complete all questions and provide their first and last name to have a valid entry and receive credit for the entry.
Prize Drawing: On or about November 17, 2023, WoodmenLife will select the winner in a random drawing from among all eligible entries received. The odds of being selected depend on the number of entries received. WoodmenLife will attempt to notify the winner via telephone or email on or about December 1, 2023.
Winner: The winner will not receive the prize; rather, the winner’s respective WoodmenLife chapter will receive the prize. The winner’s chapter must be a valid subordinate body of WoodmenLife in good standing at the time the prize is paid. Under no circumstances will the prize be paid directly to the winner of the contest.
Winner Notification: The winner will be notified by email, mail, or phone. If a winner cannot be contacted or fails to respond to notification requests, or if the contact is returned as undeliverable, the winner’s WoodmenLife chapter forfeits the prize. In the event that a winner is disqualified for any reason, or the prize is forfeited, WoodmenLife will award the applicable prize to an alternate winner and their respective WoodmenLife chapter by random drawing from among all remaining eligible entries. Alternate drawings will be held until the prize is awarded. All taxes and expenses, if any, relating to the prize are the winning chapter’s sole responsibility.
Prizes/Odds: One (1) $1,000 prize paid to the winner’s WoodmenLife chapter. The winner does not receive the money. The winner’s WoodmenLife chapter receives the prize. The odds of winning will depend upon the total number of eligible entries received.
General Conditions: In the event that the operation, security, or administration of the Contest is impaired in any way for any reason, including, but not limited to fraud, virus, bug, worm, unauthorized human intervention or other technical problem, or in the event the Contest is unable to run as planned for any other reason, as determined by WoodmenLife, WoodmenLife may either (a) suspend the Contest to address the impairment and then resume the Contest in a manner that best conforms to the spirit of the Official Rules or (b) terminate the Contest and award the prize at random from among the eligible entries up to the time of impairment. WoodmenLife reserves the right to disqualify an individual it finds to be tampering with the entry process or the operation of the Contest or to be acting in violation of the Official Rules.
Winners List: To request the name of the winner and their respective WoodmenLife chapter, send a self-addressed, stamped envelope with your request to WoodmenLife Winners List, PO Box 35903, Cleveland, OH 44135. Request must be received by November 3, 2023.
